The value of vision

Objective: The value that both, healthy as well as impaired persons attribute to the ability of vision is assumed to be very high. A method to quantify visual quality of life is necessary in order to verify this assumption. This method should also allow to compare visual quality of life with such of systemic diseases.
Methods: The literature search was done with PubMed of the National Library of Medicine (www.ncbi.nlm.nih.gov; date: 06.02.2006). Search terms: 1.) value based medicine and quality of life and eye / vision / visual, 2.) time trade off / standard gamble and eye / vision / visual. Additionally, manual research was done.
Results: The value of vision for blind and visually impaired persons is quantified by the time trade off method. Patients, with a vision of 20/30-20/50 would be willing to pay 19% of their life time, if they could get back normal acuity. Patients, with a vision of 20/200-20/400 would pay 48% and blind people 60% of the rest of their life. Comparable utilities are seen by patients, who have AIDS (21%), patients with a stroke unable to walk without assistance and unable to wash themselves (46%) and patients bedridden and incontinent due to a stroke (66%). Utilities from the general public are similar.
Conclusions: Patients and the general public attach great importance to the value of vision. The time trade off method is appropriate to quantify visual quality of life and can be used to compare utilities of different diseases.
