Scheimpflug imaging in corneal refractive surgery – Keratograph versus Pentacam® before and after myopic laser in situ keratomileusis

Objective: To determine the corneal refractive power is a main focus in corneal refractive surgery. To evaluate the central corneal refractive power, Scheimpflug- topographic systems seem to be superior to the Placido-systems. The purpose of this study was to compare the corneal refractive power, using two different topographic systems (Scheimpflug- , Placido Imaging) in patients before and after myopic Laser in situ Keratomileusis (LASIK).
Methods: In 30 patients (59 eyes), mean 37,63±7,40 years (range 23-50 years) the corneal refractive power was measured before, 3 months and 6 months after myopic Lasik (spheric equivalent in mean 4,41D±1, 92; range -1,25D to -10,13D; astigmatism<1,5D) using the Pentacam® (Oculus, Wetzlar, Germany) and the Keratograph (Oculus, Wetzlar, Germany). The corneal refractive power was estimated central, in 2mm- and 4mm radial periphery at 65°, 135°, 225° and 315°. Results were reviewed statistically to significance (Wilcoxon-Rang- test p<0, 05).
Results: With both topographic systems, the corneal refractive power (D) could be achieved without any problems. Khorizontal/Kvertical pre-LASIK: Pentacam (central: 43,33D±1,38; 2mm: 43,28D±1,44; 4mm: 41,66D±1,39); Keratograph (central: 44,19D±1,37; 2mm: 44,33D±1,36; 4mm: 44,16D±1,33). Significance central, 2mm, 4mm: p<0,001. Khorizontal/Kvertckal 3 months post-LASIK: Pentacam (central: 40,08D±1,86; 2mm: 40,35D±1,68; 4mm: 41,10D±1,44); Keratograph (central: 40,90D±1,87; 2mm: 41,06D±1,92; 4mm: 41,02D±1,86). Significance central, 2mm: p<0,001; 4mm: p=0,44. Khorizontal /Kvertikal 6 months post-LASIK: Pentacam (central: 40,39D±1,81; 2mm: 40,66D±1,68; 4mm: 41,58D±1,36); Keratograph (central: 40,94D±1,72; 2mm: 41,08D±1,86; 4mm: 41,02D±1,80). Significance central, 2mm: p<0,001; 4mm: p=0,12.
Conclusions: The difference of the results, taken with both topographic systems, was highly significant in the central area and the 2mm radial periphery. Decreasing the central refractive power after myopic Lasik, does not reveal in a significant influence on the results. Due to the interpolation of the raw data in the central area, using the Placido-systems, the data have to be estimated carefully.
