Objective: To evaluate the refractive outcome and change of higher order aberrations (HOA) of secondary LASIK treatments with topo-guided customized segmental ablation using the NAVEX platform. Methods: Customized LASIK retreatments were performed on 32 eyes, mainly due to patients complaints about double images, halos, glare and night vision problems. The average ablation depth was 8.8µm and was predominantly segmental multipoint ablation. The pre-op SEQ was -1.11 D ±0.55D StDev, showing only little need for spherical refractive correction. The average pre-op MTF was 0.34, the average wavefront error 0.725µm. The average UCVA pre-op was 0.63, the BSCVA 1.0. Results: 3 months after treatment the manifest SEQ was -0.13D ±0.38D StDev. The UCVA improved to 0.9, the BSCVA remained at 1.0. The average MTF was 0.38 increasing significantly, the wavefront error decreased to 0.63µm. Conclusions: The treatments showed a broad spectrum of success ranging from gain of more than 2 lines up to loss of a line. Although in some cases objective measurements (wavefront map, refraction, point spread function, topography) showed an improvement, the patients had the impression of no effect at all.
