The living parasite in the anterior chamber (Case report)

Objective: The acute iridocyclitis associated with parasite infection is despite of increasing tourism and migration a very rare in middle Europe.
Results: The authors report a case of 14-year-old boy presented to the Eye Department of University Hospital in Olomouc with the diagnosis of acute anterior uveitis. A living parasite could be detected in the anterior chamber in a slit lamp examination. The vitreous and retina remained uninvolved. The patient underwent surgical removal of the parasite via paracentesis and topic treatment with Atropine and Dexamethasone drops. Histopathological examination revealed the parasite as a larval stadium of Taenia (sparganum). Based on the analysis of development cycles of different types of Teania and according the literature review regarding possible parasitological etiology in the region of Czech Republic genus Spiromera seemed to be the most plausible cause of the disease in our patient. As the most probable source of viable parasites authors suspect swimming in the dam in the region of Moravia.
Conclusions: This case report shows that also parasite disease should be kept in mind as a course for acute iridocyclitis. Isolated localization of a parasite in the anterior chamber is very rare and has a good prognosis for the visual acuity.
