SO.05.03 Primary vitrectomy with intraocular antibiotics application in endophthalmitis Rehak M., Meier P., Wiedemann P. University Eye Clinic, Leipzig Objective: The pars-plana vitrrectomy with intravitreal antibiotics application is establisched procedure for treatment of endophthalmitis. The presented study analyzed our own results with this treatment. Methods: We analyzed the data of 56 consecutive patients from January 2000 to March 2006. All patients underwent vitrectomy and intravitreal antibiotics application. The effect of vitrectomy with respect to final visual acuity and the rate of postoperative complications were analyzed. The vitreous was microbiologically examined. Postoperative follow-up time ranged from 2 weeks to 24 months. Results: The cases of endophthalmitis followed cataract surgeries with intraocular lens implantation in (55% of patients), perforated corneal ulceration (16%), traumatic endophthalmitis (9%), pars-plana vitrectomy (7%) and glaucoma-surgery (4%). Five patients (9%) with endogenous endophthalmitis were included in the study. Positive cultures were obtained in 50% of patients. Visual outcomes: At the final examination 68% of patients gained visual acuity of 20/400 or better, 21% of patients left with visual acuity of light perception and 6 eyes were enucleated. Conclusions: Immediate vitrectomy in combination with intraocular antibiotics administration is an effective method for the treatment of endophthalmitis resulting in preservation of ambulatory vision in most of the patients.
