Intravitreal Bevacizumab as monotherapy or in combination with photodynamic therapy in the treatment of choroidal neovascularization secondary to Pseudoxanthoma elasticum

Objective: To evaluate functional and morphologic results of intravitreal Bevacizumab (Avastin) as a monotherapy or in combination with photodynamic therapy (PDT) in the treatment of choroidal neovascularization (CNV) secondary to pseudoxanthoma elasticum (PXE).
Methods: Eight patients with CNV in PXE were treated with a single injection of 1,5mg intravitreal Bevacizumab. Two patients received additional Verteperofin Photodynamic therapy. Within one hour after PDT patients received an intravitreal injection of Bevacizumab. At baseline as well as 4, 8 and 12 weeks after treatment best corrected visual acuity was obtained and patients were examined by optical coherence tomography (OCT) and fluorescein angiography.
Results: Three patients improved more than three after twelve weeks but no patient lost more than 3 lines after twelve weeks. Mean retinal thickness determined by OCT decreased by 110 µm 12 weeks after treatment. Fluorescein angiography displayed a marked reduction of hyperfluorescence in the CNV area. A second injection of intravitreal Bevacizumab was performed in three patients. No complications occured in any of the patients.
Conclusions: Intravitreal Bevacizumab as a monotherapy or in combination with PDT seems to be a promising and safe treatment modality for patients with CNV secondary to angioid streaks in PXE.
