DO.01.13 Unchanged refraction of a new meniscus lens in contact with vitreous substitutes. Report on the first implantation series in eyes filled with silicone oil Petersen J., Baraki H., Ritzau-Tondrow U. Department of Ophthalmology, University Göttingen Objective: The higher refractive index of silicone compared to water causes considerable hyperopic shift in phakic and pseudophakic eyes. The consequences are deminished visual acuity, anisometropia and anisoeikonia. A new lens design im combination with a modified implantation technique is able to avoid these problems completely. Methods: We designed a posterior chamber lens with concave posterior surface and center of curvature on the macula (Typ ACRITEC Ret 59). The deflection of light takes place completely at the anterior surface of the lens. The posterior surface is transmitted at right angles. Therefore a modified refractive index inside the vitreous cavity does not alter the focus. The haptics are implanted into the capsular bag, the optical part is pushed through a window of the posterior capsule into the vitreous cavity. By that means an optical perfect contact between lens and oil is guaranteed. Results: The lens has been implanted into 86 eyes in combination with vitrectomy and silicone oil tamponade. The postoperative refraction coincided with the expected values computed by the usual formulas (for water!). On an average there was no refractive shift after oil removal. Conclusions: The new lens type enhances visual rehabilitation essentially for those patients, who need transitory or definite vitreous substitution or must be expected to need it in the future. The main target group are patients with late diabetic complications of the retina.
