104. Jahrestagung der Deutschen Ophthalmologischen Gesellschaft 2006 Informationen für Teilnehmer

Die Mitgliedschaft in der DOG bzw. die Bestätigung eines Referates, Vortrages, Videos
oder Posters beinhaltet nicht automatisch die Anmeldung zur Jahrestagung.
DOG-Mitglieder, Referenten und Vortragende werden daher gebeten, sich rechtzeitig zur
Tagung anzumelden, um einen reibungslosen Ablauf zu gewährleisten.
Die Anmeldung kann vorzugsweise online erfolgen.
Alternativ können Sie ein
Anmeldeformular (PDF) herunterladen, ausfüllen und per Post oder
Fax an das Kongressbüro senden.
Alle Mitglieder der DOG, die sich bis zum 25. August 2006 angemeldet haben sowie alle
Nicht-Mitglieder, die bis zu diesem Termin ihre Teilnahmegebühr entrichtet haben
(Zahlungseingang auf dem Konto), erhalten ihr Namensschild und einen Gutschein für
die Kongressunterlagen per Post. Für die Zweitausstellung eines Namensschildes vor Ort
wird eine Verwaltungsgebühr in Höhe von EUR 15 erhoben.
Anmeldeschluss ist der 15. September 2006, danach nur noch bei der Tagung im Maritim!
Membership of the DOG or confirmation of any oral presentation, video or poster does
not automatically include the registration for the annual meeting.
Therefore, all members of the DOG and all speakers are requested to register prior to the
conference.
Online registration is preferred.
In case you
wish to register by mail or fax, the respective form can be downloaded and should be sent to the congress office.
All members as well as non-members of the DOG, who have registered and paid their
registration fees by 25 August 2006 will receive their name tags and a voucher for the
congress documents by postal mail. There will be a processing fee of EUR 15 for a second
name tag at the congress. Registration deadline: 15 September 2006, afterwards
registration is possible exclusively on site at the Maritim Hotel!

Stornierungsbedingungen
Terms of cancellation
Stornierungen werden ausschließlich in schriftlicher Form akzeptiert. Bei Stornierung bis zum 19. Juli 2006 erfolgt eine Rückerstattung abzüglich einer Bearbeitungsgebühr
von EUR 25. Nach diesem Termin erfolgt keine Rückerstattung.
Cancellations of registration will only be accepted in writing. In case of cancellation by
19 July 2006 registration fees will be refunded minus a handling fee of EUR 25. After this
date there will be no refund.
Porstmann Kongresse GmbH tritt in jedem Fall nur als Vermittler auf und haftet nicht
Liability für Verluste, Unfälle oder Schäden an Personen oder Sachen, gleich welchen Ursprungs. Die Haftung der mit der Leistungserbringung beauftragten Personen oder Unternehmen
bleibt davon unberührt. Mündliche Absprachen bedürfen einer schriftlichen Bestätigung.
Ausschließlicher Gerichtsstand ist Berlin.
Mit seiner Unterschrift auf dem Anmeldeformular erkennt der Teilnehmer die
genannten Bedingungen an.

liability of persons or companies charged with providing the relevant service.
Oral arrangements require a written confirmation. The sole court of jurisdiction is Berlin.
With his signature on the registration form the participant accepts the above
mentioned conditions.
